Modafinil vs Caffeine: How the Research Compares, and What It Felt Like

In a modafinil vs caffeine sleep-deprivation trial, both improved vigilance and performance against placebo, and modafinil's effect lasted longer.3 Both can increase blood pressure and heart rate, so combining them may enhance those effects.1 In my own use, caffeine timing mattered more than I expected.

The sleep-deprivation studies

The trial that separates the two on performance is a randomized controlled trial in Royal Netherlands Air Force crewmembers, published in the Journal of Psychopharmacology.3 Thirty-two volunteers took part, double-blind, on three non-consecutive trial days: 200 mg modafinil, 300 mg caffeine, or placebo, given just after midnight, after a median 17 hours awake, followed by six series of the Vigilance and Tracking test, the psychomotor vigilance task and the Stanford Sleepiness Scale.3

Both treatments produced statistically significant effects against placebo on every endpoint except the mean tracking error of the Vigilance and Tracking test.3 The authors' conclusion is that both improved vigilance and performance, that both lowered reported sleepiness after a limited period of sleep deprivation, and that modafinil was effective for longer than caffeine.3 The trial separated them at 8 hours after administration: modafinil significantly improved the psychomotor vigilance and sleepiness scores compared with caffeine.3

The scope is limited: a night-time vigilance study after about 17 hours awake, not rested volunteers.3

The second study takes a different angle and lands somewhere more cautious. It is a double-blind pilot published in Sleep Medicine in 2024, using brain imaging across three 36-hour sleep-deprivation trials, with participants taking 200 mg of caffeine, modafinil or placebo at the 28th and 32nd hours of wakefulness.4 Subjective sleepiness increased with the duration of sleep deprivation, and compared with placebo, modafinil and caffeine had no significant effect on wakefulness or sleepiness in that pilot, even though the imaging showed the two conditions changing connectivity between the thalamus and other brain regions.4 The broader claim sits in the paper's background section, attributed to earlier studies: caffeine and modafinil can alleviate cognitive impairment from sleep deprivation "to some extent."4

How long each lasts

The trial also reported a difference in duration.

Modafinil was effective for longer than caffeine, which the authors describe as consistent with its longer half-life.3 Both treatments showed an effect against placebo on the psychomotor vigilance results from 2 hours after administration, and on the VigTrack parameters and sleepiness scores 2 hours after that.3 At 8 hours, modafinil significantly improved the psychomotor vigilance and sleepiness scores compared with caffeine.3

Combining them: what the interaction checkers say

Both interaction sources cover the combination, and both rate modafinil with caffeine as a moderate interaction.12

Drugs.com states the concern directly. Both caffeine and modafinil can increase blood pressure and heart rate, and combining them may enhance those effects.1 The same page carries a therapeutic duplication warning: the recommended maximum number of medicines in the stimulants category to be taken concurrently is "usually one", and caffeine and modafinil are both listed in that category and in the cerebral stimulants category.1 It adds that a doctor is the right person to ask before the two are used together, particularly with a history of high blood pressure or heart disease.1

WellRx lists caffeine-containing foods and beverages as a moderate interaction with modafinil. Its consumer note advises that excessive caffeine intake be limited while modafinil is being taken, and records that a high intake can increase the risk of nausea, nervousness, palpitations, problems with sleep and a rapid heartbeat.2 Its professional note reads: caffeine is a CNS stimulant, and those actions "are expected to be additive when coadministered with other CNS stimulants or psychostimulants."2

One long-term user's experience with coffee and modafinil

In the early years I stacked several coffees on top of modafinil and could not sleep until late. At the time I assumed that was just the cost.

Later I kept the coffee to much less, and not until well after the effect had started. The jittery edge went away.

Energy drinks made things worse for me: a racing heart felt more tiring, not less. A bad mix on an already anxious morning amplified my anxiety.

That is one person's pattern, and nothing beyond it.

Which suits what

Modafinil treats sleep disorders such as narcolepsy, obstructive sleep apnea and shift work disorder,2 and in the Wingelaar-Jagt trial its effect lasted longer.3 WellRx lists caffeine under caffeine-containing foods and beverages, with coffee, teas, colas, chocolate and some herbal supplements among the examples it gives,2 and in the same trial its effect did not last as long as modafinil's.3

The interaction listings cover the combination rather than the comparison: both rate it moderate,12 and the duplication note counts the pair in the same categories.1 Drugs.com says combining them may enhance their effects on blood pressure and heart rate,1 and WellRx describes the stimulant actions as expected to be additive.2
