Benefits of Modafinil: What the Research Supports and What Stays Unproven
The best-supported benefits of modafinil are wakefulness in the sleep disorders it is used to treat, better performance under sleep deprivation, and modest gains on complex thinking in healthy people.61345 The evidence is weaker for everything else.

Benefit 1: What does modafinil do in the conditions it is used to treat?
Modafinil is used to treat narcolepsy, sleep apnea and shift-work sleep disorder, according to Wikipedia's entry on the drug.6 One regulatory change belongs alongside that list. The Mayer paper's own background records that in 2010 the European Medicines Agency withdrew the indication of modafinil for obstructive sleep apnea, shift-work sleep disorder and idiopathic hypersomnia.2
In shift-work sleep disorder, patients chronically have excessive sleepiness during night work and insomnia when attempting to sleep during the day.1 A three-month, double-blind trial randomly assigned 209 patients with that disorder to receive either 200 mg of modafinil or placebo before the start of each shift, with monthly assessments using a nighttime sleep-latency test, a clinician rating of change, a vigilance test, diaries of patients and daytime sleep monitoring.1 More patients on modafinil improved in their clinical symptoms, 74 percent against 36 percent on placebo.1
Nighttime sleep latency improved by about 1.7 minutes on modafinil against 0.3 minutes on placebo, lapses of attention during nighttime testing fell in frequency by 2.6 on the drug while rising by 3.8 on placebo, and 29 percent of patients on modafinil reported accidents or near accidents while commuting home, against 54 percent on placebo.1 Daytime sleep was not worse on modafinil than on placebo.1 The authors called the sleep-latency improvement modest and the overall performance result small but significant compared with placebo.1
Outside the uses above: idiopathic hypersomnia
A later randomized, double-blind, placebo-controlled study tested a condition that is not among the uses listed above, idiopathic hypersomnia without long sleep time.26 The study recruited 33 participants, who received either 100 mg of modafinil in the morning and at noon for three weeks or placebo.2 Compared with placebo, modafinil decreased sleepiness significantly, and the participants' diaries showed significant improvement in performance and in exhaustion.2 The change in mean sleep latency in that study did not reach significance.2
Benefit 2: performance while sleep-deprived
The sleep-deprivation performance results in this page's sources come from studies of pilots, and this page uses them as research, not as practice. A 2004 study used 100 mg, given after 17, 22 and 27 hours without sleep, and tested ten Air Force F-117 pilots in a quasi-experimental, single-blind, counterbalanced design across 37 hours of continuous wakefulness.3
The paper's abstract records why it was run: modafinil had been well tested in clinical settings, and further studies were needed to establish its safety and efficacy for use in pilots.3
Modafinil attenuated flight performance decrements on six of eight simulator maneuvers.3 It held flight accuracy within roughly 15 to 30 percent of baseline, while performance in the no-treatment/placebo condition fell by as much as 60 to 100 percent.3 It also lowered self-rated depression and anger and improved ratings of vigor, alertness and confidence, with the benefits most noticeable after 24 to 32 hours of continuous wakefulness.3
Two limits come from the same paper. Its conclusion says that modafinil did not sustain performance at predeprivation levels.3 It also records that, at the dose level used, the drug's effects were not subjectively salient, and warns that this could lead personnel to escalate the dose.3
A review of how modafinil works in the brain covers comparable ground. It describes a helicopter study: six pilots kept awake for two 40-hour periods, with three 200 mg doses in one period and placebo in the other, where simulated flight performance stayed near baseline on modafinil and fell by roughly 10 to 20 percent without it.5
Benefit 3: complex thinking in healthy people
A 2015 systematic review covered cognitive neuroenhancement in healthy, non-sleep-deprived subjects. It evaluated research papers on cognitive enhancement with modafinil from January 1990 to December 2014 and found 24 studies covering planning and decision making, flexibility, learning and memory, and creativity.4 Its finding is narrow: modafinil "does indeed confer significant cognitive benefits in this group, at least on a particular subset of tasks."4
The review found the benefit varied with the task.4 The longer and more complex the task tested, the more consistently modafinil helped.4 It made no difference to working memory or flexibility of thought, but it did improve decision-making and planning.4
The mechanisms review cited above is more cautious, writing that studies of neurocognitive functioning tend to confirm that modafinil mildly enhances cognitive performance in healthy volunteers, especially with regard to executive function.5 That review also records a narrower result from one group's own follow-up review, where modafinil improved performance in average IQ subjects but not high IQ subjects, and reports that those authors concluded modafinil has limited cognitive enhancing effects in already high-performing, well-rested people.5
One proposed mechanism
Gerrard and Malcolm report that modafinil did not increase speed of response at the cost of accuracy, but it increased accuracy while reducing information processing and response time, which the review reads as a sign that it may also enhance neural efficiency.5
Claimed benefits with weak evidence
Several claims about modafinil benefits are not supported well enough to be called benefits.
- Research: multiple sclerosis fatigue. Wikipedia's entry on the drug reports that improvements in fatigue severity scores have not consistently reached statistical significance, though reviews and meta-analyses of controlled trials have found modest effectiveness for fatigue in multiple sclerosis, and most multiple sclerosis organizations are neutral on the off-label use.6
- Research: adult ADHD. Wikipedia's entry notes that modafinil is occasionally prescribed off-label in attention deficit hyperactivity disorder, that it has not consistently shown efficacy in treating adult ADHD, that the evidence in adults is mixed, and that a 2016 systematic review did not recommend its use.6
- Research: depression. Wikipedia's entry reports that systematic reviews and meta-analyses found the evidence limited, the quality of that evidence low and the results inconclusive, and that modafinil did not significantly improve depression in a network meta-analysis, though there was some evidence for reduced fatigue and sleepiness.6
Those are unproven uses rather than benefits, and they say nothing about what modafinil does in the conditions above. This page does not present them as reasons to use it.
What the benefits felt like in practice
On the first day, I noticed clearer thinking.
Over the years, what stood out for me was reliability rather than a buzz. It did most for long, defined tasks that were set out in advance.
For me, sleep was the boundary. After a poor night, the benefits disappeared, and across those years the benefit depended on the task and on how I had slept. The 2004 study of pilots found that modafinil reduced the performance decline on most simulator maneuvers under sleep deprivation, although performance remained below predeprivation levels.3

The benefits have limits
- Research: In the 2004 study of pilots, the authors concluded that modafinil did not sustain performance at predeprivation levels.3
- Research: In the shift-work trial, patients on modafinil continued to have excessive sleepiness and impaired performance at night, and the authors wrote that the residual sleepiness underscores the need for interventions that are even more effective.1
- Research: In the healthy-volunteer review, the benefit applied to a particular subset of tasks, while working memory and flexibility of thought were unchanged.4
- Research: In the mechanisms review, the effect in healthy volunteers is described as mild, the authors of the follow-up review concluded modafinil has limited cognitive enhancing effects in already high-performing, well-rested people, and the drug's primary mechanism of action remains elusive.5
