Modafinil Pros and Cons: What the Research Supports and What It Doesn't Fix
The modafinil pros and cons, in short: research supports better performance on complex tasks and against sleep deprivation, headache is very common at up to 34 percent, and long-term safety is not known.1234 In my experience, it amplified bad aims and bad moods as readily as good ones.

What the research supports
The cognition pros rest on a systematic review, described in a 2015 University of Oxford release that is careful about how far the findings reach. The release says its authors evaluated all research papers on cognitive enhancement with modafinil from January 1990 to December 2014 and found 24 studies, covering planning and decision making, flexibility, learning and memory, and creativity.1 Its headline is qualified: modafinil "does indeed confer significant cognitive benefits in this group, at least on a particular subset of tasks".1
"What emerged was that the longer and more complex the task tested, the more consistently modafinil conferred cognitive benefits."1 Modafinil "made no difference to working memory, or flexibility of thought, but did improve decision-making and planning". The release ties the pattern to the tests used: "In the past, people were using very basic tests of cognition, developed for neurologically-impaired individuals. In contrast, more recent studies have, in general, used more complex tests: when these are used, it appears that modafinil more reliably enhances cognition: in particular 'higher' brain functions that rely on contribution from multiple simple cognitive processes."1
The Alzheimer's Drug Discovery Foundation describes a systematic review's results by task type. Most studies suggest modafinil does not improve simple cognitive functions such as attention, verbal working memory and cognitive flexibility, possibly because participants already had high baseline scores on those tests. Effects on learning and memory are mixed, most studies suggest it does not affect creativity, and some suggest it may impair it. It "may improve some aspects of executive function such as planning, decision-making, and fluid intelligence", and improved performance on complex tasks rested on five clinical studies within the review.3
Sleep deprivation is where the case becomes concrete. A 2004 study tested modafinil in 10 Air Force F-117 pilots across 37 hours of continuous wakefulness, in a quasi-experimental, single-blind, counterbalanced design.2 Modafinil "attenuated flight performance decrements on six of eight simulator maneuvers", holding flight accuracy "within approximately 15-30% of baseline levels", while performance in the no-treatment condition "declined by as much as 60-100%".2 Mood moved with it: self-ratings of depression and anger fell, and ratings of vigor, alertness and confidence rose, with the benefits "most noticeable after 24 to 32 h of continuous wakefulness".2
The same paper states the ceiling, and it belongs beside the results: "modafinil did not sustain performance at predeprivation levels".2 Both statements describe one study: performance was not sustained at pre-deprivation levels, and it held far closer to rested levels than it did without the drug.2
What the research doesn't know
The blunt version comes from the Alzheimer's Drug Discovery Foundation, in a section headed Safety: "The long-term safety of modafinil is not known."3 The same source frames the cognitive question narrowly. Modafinil may improve cognitive performance on complex tasks in healthy individuals, "though no studies have examined it for long-term brain health", and no studies have examined whether it prevents cognitive decline or dementia.3
The 2015 Oxford release reports what the studies found on mood and side effects: "the 70% of studies that looked at the effects of modafinil on mood and side effects showed very little overall effect, although a couple reported insomnia, headache, stomach ache or nausea (which were also reported in the placebo group)."1 The release places the review in a non-sleep-deprived group: it calls it "the first overview of modafinil's actions in non-sleep-deprived individuals since 2008", and says its side-effect findings came from "these controlled environments".1
Dependence sits in the same gap. A 2015 case report describes one patient, who also had another psychiatric diagnosis, and who increased the amount over a longer period and met the criteria for dependence, with tolerance, withdrawal, inability to cut down, progressive increase and craving all recorded.5 The report's title calls its case a rare one, and it adds that while "abuse potential is claimed to be less, it should be considered serious", because of rising use among young people.5 The Alzheimer's Drug Discovery Foundation puts that in context: "Modafinil is preferred to other wakefulness-promoting drugs due to its lower risk of addiction, though it is still a schedule IV-controlled substance due to rare case reports of abuse."3
Pros after more than a decade (experience)
After more than a decade, the pros list was short.
The first was dependable focus I found on a defined, solo, deep task: one piece of work, decided in advance, with no conversation attached.
The second was that tedious, repetitive work became bearable.
The third was appetite suppression. It came as a side effect, and it was useful when I was deliberately controlling my weight. It sits on both lists here, because the same effect produced the skipped meals described below.
More than a decade in, that was the honest total: attention that landed on the decided task, tedious work that stopped being a fight, and less appetite. It was the same work with less friction in it.

The cons list was longer than the pros list.
Without a plan, I focused on the wrong thing. The attention was real; the target was whatever happened to be in front of me.
It also made me more focused and less warm socially. Days built around conversations were the days I left it alone.
It amplified my anxiety on stressed mornings. The caution section below returns to it.
Appetite suppression had a second edge: it led to skipped meals and flat afternoons.
I experienced tolerance with daily use. Drugs.com lists tolerance among postmarketing reports. The listing gives no frequency and does not establish that tolerance is inevitable.4

What it doesn't fix
Four things stayed outside its reach.
Sleep is the first (experience). In my experience, it did not fix poor sleep. The closest the research comes is the simulator study: "modafinil did not sustain performance at predeprivation levels".2 The drug has its own cost at the sleep end, with insomnia listed among the psychiatric adverse reactions reported for 1 to 10 percent of patients.4 The Oxford review adds that a couple of the studies it covered reported insomnia, which was also reported in the placebo group.1
The second is the wrong goal (experience). My attention went to whatever was being worked on, and without a plan I focused on the wrong thing.
Anxiety is the third (experience). It did not fix my anxiety; on stressed mornings it made it worse. The research side is narrower than that: Drugs.com lists anxiety among the more common side effects and among the psychiatric adverse reactions reported for 1 to 10 percent of patients, alongside nervousness, insomnia, depression and emotional lability (research)4.
Social skills are the fourth (experience). For me it did not fix them; it made me more focused and less warm socially.
Who I would flag for caution
Three groups stood out for me.
The first is anyone using it as a cognitive enhancer outside its approved use (experience). The 2015 Oxford release is explicit: "modafinil is not licensed for this use, and it will not be because it would be outside the current terms of reference of regulatory bodies." (research)1 The Alzheimer's Drug Discovery Foundation describes it as "still a schedule IV-controlled substance due to rare case reports of abuse" (research).3
The second is anyone whose anxiety is already close to the surface (experience). Drugs.com lists anxiety among the more common side effects and among the psychiatric adverse reactions reported for 1 to 10 percent of patients (research).4 In my own account, it was worse on stressed mornings.
The third is anyone short of sleep (experience). In the simulator study, performance was not sustained at pre-deprivation levels, and insomnia sits on the common list (research).24 In my experience, it did not fix poor sleep.
